Further Details – these questions relate to the Adult at Risk
You are completing this section of the form on behalf of the Adult at Risk, so unless you are the Adult at Risk, you should select ‘Someone else' or ‘Someone else in a professional capacity.'
The questions which follow relate to the Name, Date of Birth, Gender, Ethnicity and Address Details of the Adult at Risk. The system requires a date of birth for the Adult at Risk to be added. If you do not know the Adult's date of birth select estimate DOB and add in 01/01/1900.
Saving your form to complete later
Once your form has been saved but NOT submitted to Knowsley MB Council. You have 24 hrs to complete and submit this form.
Safeguarding Concerns must be reported
IMMEDIATELY in order to safeguard the Adult at Risk.
