Information to be aware of: • If you don't have an up-to-date Social Care Needs Assessment (which is a criteria for an applicant to be added to the Extra Care waiting list) the Extra Care Team will request one on your behalf, and someone from Knowsley's Adult Social Care Team will be in touch with you to arrange a date and time for an assessment once you have been allocated - (an up to date Social Care Needs Assessment would be within the last 12 months) • Once you have had your Social Care Needs Assessment you will be added to the Extra Care waiting list. • Please be aware that Extra Care is a tenancy for life and therefore there may be a lengthy wait for a vacancy to become available. • Please note you will not be contacted until a vacancy is available in the scheme of your choice. • It will not be possible to advise you on where you are on the waiting list as individuals are prioritised once a vacancy becomes available, therefore the priority may change as new individuals are added to the waiting list. • If your care needs have changed since your last Social Care Needs Assessment, and you now feel you may need additional help with your care, you should contact Adult Social Care on 0151 443 2600. They will not be able to discuss your Extra Care application but can assist you if your care needs have changed.
