Extra Care Referral

In order to access Extra Care accommodation you will be required to:

• Complete your Extra Care Application.
• Register onto Property Pool Plus, provide your proof documents and any other relevant documents for your application to reviewed and your housing band awarded.

N.B to be eligible for Extra Care accommodation, the policy states an applicant must have been living in the borough of Knowsley continuously for the last 12 months. Please complete all sections of your Extra Care Application and provide the most relevant and up to date information which will assist with the processing and assessment of your accommodation needs.   In some circumstances, we may need to request further information from you, or from an agency that is supporting you to fully assess your needs and ensure that you are provided with the most appropriate advice and guidance.

We ask that you carefully consider which Extra Care schemes you wish to be considered for and you will be asked to indicate your two preferred areas within Knowsley. In order to complete your application for Extra Care we also require you to register yourself with Property Pool Plus so that you can be awarded a housing banding allowing you to be prioritised dependant on your assessed housing need. You can do this via:www.propertypoolplus.org.uk/Home.

When you have registered with property pool plus and have been awarded your housing band, you can then submit your application to Extra Care. Knowsley Council will process your Extra Care application and you will then be eligible to be nominated for any future vacancies then all information will be submitted to the Extra Care nomination panel who will confirm your eligibility. If you are deemed eligible for Extra Care accommodation you will then be nominated based on your priority status and subject to a property becoming available within one of our schemes.

Where it is assessed that you are not eligible for Extra Care accommodation, we will provide advice on your alternative housing options if required.
