This review document is intended to be used for carers whose needs remain the same as they were in the last assessment.

It is intended to make the review process for carers more efficient to make sure they get the support they need as quickly as possible.

It should be used in most cases, a change in the cared for persons needs does not necessarily indicate a change in the carers needs.

Disclaimer for Carer's Personal Budgets

This Carer's Personal Budget payment is provided to support you in your caring role and must be used in line with your agreed Carer's Support Plan.

The budget must only be spent on the activities, services, or items that have been approved as part of your support plan.

You may be asked to provide evidence of how the budget has been used. Therefore, please retain all receipts and relevant records of expenditure for a minimum of 12 months from the date of purchase.

This will help ensure that the funding has been used appropriately and in accordance with the agreed purpose.
